Reject copy button when a copy is supplied by requester

When a requester supplies their own scan sometimes it's not appropriate because it's an ILL copy or it isn't a cleanly scanned copy, it would be good to have the option to just reject the copy rather than have to reject the whole request.

There are currently 'previous' and 'next' page buttons at the bottom of the requests page in TADC. But it would also be helpful to have them at the top to save scrolling.
